📋 Instructions for Submitting Medical Reimbursement Claims

1. Submit Form 7 along with all original bills and prescriptions.
2. Claim must be submitted within 6 months from the date of treatment.
3. In emergencies, private hospital bills are reimbursable even if not empanelled (with justification/approval).
4. Permission & estimate required in advance for planned surgeries.
5. Keep photocopies of all submitted documents.
6. Prefer treatment at CGHS/ESI/State Government empanelled hospitals.
